Assistant Family Services Coordinator (Assistant IDD Case Manager)

Peaceful Living is seeking an Assistant Family Services Coordinator to oversee the provision of Home and Community Based Services to individuals with intellectual and developmental disabilities on their assigned caseload. In addition, the Assistant Family Services Coordinator will provide Habilitation and Community Participation services when assigned by the Family Services Manager.

Location: Harleysville, PA First-shift, Monday-Friday, some additional hours as assigned Full-time

  • Be at least 18 years of age
  • Possess a high school diploma/GED
  • Have and maintain a valid (state of residence) driver’s license and driving approval from Peaceful Living
  • Successfully pass and maintain all criminal background checks
  • Complete pre-employment physical and TB test
  • Attend a week long New Employee Orientation held at our main office in Harleysville from 9am – 4pm.
  • Be fully vaccinated and receive a booster when applicable. If not vaccinated, you may show proof of upcoming vaccine appointment to fulfill this requirement.
  • N95 mask fit testing, provided by Peaceful Living
  • Will attend and obtain certification in the PA Certified Investigator’s Course within one year of date of hire
  • High school diploma or equivalent required. Associates degree or bachelor’s degree in related field preferred; a minimum of one year of experience working within the field of intellectual and developmental disabilities

Key Responsibilities:

  • Directly oversee a caseload of individuals; Complete regulatory requirements such as compiling progress notes, monitoring service utilization, developing outcomes, and attending ISP meetings.
  • Maintains professionalism when interacting with individuals and families and external stakeholders
  • Supervise Direct Support Professionals (DSPs) while fostering their professional development, maintain clear communication on job duties and scheduling, and meet for regular 1:1 supervisory meetings. Provide performance coaching as appropriate.
  • Maintain schedules for assigned caseload and share On Call duties.
  • Participates in weekly team meetings, and implements departmental procedures and initiatives
  • Provides direct support to individuals as appropriate, in order to maintain service ratios and foster ground level understanding of individual programming and ensure quality care and support
  • Take initiative on learning best practices and skills related to field
  • Collaborate with community organizations to develop innovative volunteer, social and recreational activities for individuals and provide transportation as appropriate
  • Transport individuals, at times using personal vehicle, to Outcome and Goal related activities that are within the departmental mileage cap
  • Respond appropriately to emergency situations.
